HEALTHY FOR LIFE PROGRAM

 

Our Healthy for Life Program is an Australian Government Program that is aimed at helping Aboriginal and Torres Strait Islander people to get better health. Trained Aboriginal Health Workers are available to provide guidance and health care with child development, children health, immunisations child/maternal health, men’s health, women’s health, diabetes and management of chronic disease.

 

The Healthy for Life team assist with the follow up from visiting specialists, these include: Podiatrist, Optometrist, Cardiologist, Physiotherapist, Occupational Therapist, Endocrinologist, Haematologist, and Respiratory professionals. At Healthy for Life we continue to work alongside the GP's and Visiting specialists by following up with client's to provide extra support to attend their appointments.

Program Objectives are to:

Ø  Enhance the quality of life for Aboriginal and Torres Strait Islander people with chronic and complex illness

Ø  Improve the health of mothers, babies and children

Ø  Improve the long term health outcomes for our clients

Ø  Reduce the incidence of adult chronic illness over time

Ø  Healthy for Life’s program design is based on delivering population health approaches in a primary health care context using quality improvement principles, processes and tools.

 

How can the Healthy for Life Team help you?

Our Aboriginal Healthy for Life Team is made up of Registered Nurses, a Midwife and Health Workers who have the passion to improve the health of Aboriginal and Torres Strait Islander people

 

REGULAR PROGRAMS

H4L has instituted work practice changes to help create efficiencies within the service and the uptake of GPMP and annual cycles of care is increasing with the subsequent benefit of increased Medicare rebates to the organization.

Regular screenings include, STI. Bowel Cancer, Breast Screening. Immunizations, Trachoma and Hearing

 

GROUP ACTIVITIES

Regular programs continue to include Mothers and Babies’ play group, Bungala Sewing Group. Kupa Piti Mamaku Group. H4L also participates in a School Program
